Take the First Step Toward TMS Care

The Patient Health Questionnaire-9 (PHQ-9) asks nine questions about depression symptoms over the past two weeks. We will also ask you specific medication & treatment history questions at the end of the questionnaire. Your responses provide a starting point for a conversation about your symptoms, treatment history, and whether a TMS evaluation may be appropriate.

Interested in exploring TMS for depression? Complete the brief screening and interest form below to help our team understand what you’re experiencing and connect with you about next steps.
